From Intake to Audit, Part 1: Medical Necessity and Intake Documentation
In a solo practice, no one reviews your notes before a claim goes out. You are the counselor, the biller, the compliance officer, and the records custodian, and every claim carries your NPI. This course builds the foundation every defensible claim rests on: medical necessity and the intake. You will learn how Medicare, Medicaid (including EPSDT and managed care plan definitions), TRICARE, and commercial plans each define medical necessity, and how to document once so the record answers all of them. You will work through the six pillars of a medically necessary record, a fully worked treatment plan, continued-stay and discharge documentation, utilization review calls, and denial responses. The course then walks through intake from the first phone call: verification of benefits, payer-specific checks, consent, Good Faith Estimates, superbills, and an element-by-element guide to the 90791 diagnostic evaluation, with sample documentation throughout. Ready-to-use tools include an intake and 90791 checklist, a medical necessity worksheet and treatment plan builder, and two practice games, including Note Lab, where you write and score real notes. Part 1 of the three-part From Intake to Audit series.

What you'll learn
- Differentiate the medical necessity standards used by Medicare, Medicaid (including EPSDT and managed care plan definitions), TRICARE, and commercial plans.
- Construct documentation that links a criteria-supported diagnosis, measurable functional impairment, an individualized treatment plan, evidence-based interventions, and measured progress.
- Write continued-stay justifications, discharge summaries, and responses to "not medically necessary" denials that cite record evidence.
- Apply a pre-intake protocol covering benefit verification, payer-specific checks, informed consent, Good Faith Estimates, and superbills.
- Document and code a 90791 diagnostic evaluation that contains every required element, follows same-day and frequency rules, and avoids coding uncertain diagnoses.
Who it's for
Licensed professional counselors, licensed clinical mental health counselors, and other licensed mental health clinicians who bill third-party payers, written especially for solo practitioners who handle their own billing and compliance.
